In an interview with The Educated Patient, Jacob Teitelbaum, M.D., discusses the treatment and management of long COVID, chronic fatigue syndrome (CFS) and fibromyalgia, describing these conditions as manifestations of an “energy crisis” in the body. He emphasizes that while long COVID has significantly increased the population suffering from these conditions, effective treatment strategies are available.

Teitelbaum explains that long COVID, like CFS and fibromyalgia, results from energy deficits that disrupt the body, particularly affecting the hypothalamus and muscles. Many symptoms are rooted in this energy crisis, which is often driven by inflammation and oxidative stress.

He advocates for a personalized approach using natural remedies, lifestyle adjustments, and targeted supplements. His SHINE protocol — Sleep, Hormones, Infections/Inflammation, Nutrition and Exercise as able — addresses the core dysfunctions in these conditions:

  • Sleep: Recommends sustained-release melatonin; herbal formulas; chamomile tea; and low-dose medications, such as trazodone, to improve restorative sleep without side effects.
  • Hormones: Suggests addressing hormonal imbalances, particularly in patients with postural orthostatic tachycardia syndrome (POTS) or hypotension.
  • Inflammation: Highlights the role of curcumin and glutathione to combat inflammation and oxidative stress, key contributors to organ damage in long COVID.
  • Nutrition: Advocates for energy-boosting supplements such as HRG80 red ginseng and a multivitamin drink mix rich in B vitamins. He advises against sugar and recommends a high-protein or ketogenic diet, tailored to individual needs.
  • Exercise: Encourages gradual, low-intensity exercise like walking in sunlight, adapted to avoid post-exertional malaise. For those with severe fatigue, horizontal exercises are recommended.

Beyond physical recovery, Teitelbaum advises patients to use their regained energy for fulfilling activities aligned with their true selves, rather than returning to stressful obligations. He views recovery as an opportunity for personal growth and transformation.
